Insurance Accepted
 

UHC will be accepted at this office until January 2022.  At that time, the following self-pay rates will go into effect.  Out-of-network benefits can be used for any insurance.  The self-pay rate is paid at the time of service, and a superbill (includes the detail needed for reimbursement) will be provided, which the client sends to the insurance company.  The insurance provider reimburses the client directly according to the out-of-network benefits, which vary by plan.
